Please print and complete the form, then return to the address provided, together with your annual membership fee.
If you are employed or head of an academic department, please send us the standard membership fee, which is £30.
If you are a student, please send us the reduced student membership fee, which is £15.

On receipt of your completed form and payment fee, your membership will be processed. You will then receive a username and password by e-mail in order to login to member only areas of the Foundation website.
